Emergency

Call 112. Calling for help is required by law (German Criminal Code § 323c). levelll cannot promise immunity from criminal or administrative consequences.

Immediate

  1. 1. Call 112 if breathing, consciousness, or circulation are affected, or if the person becomes panicked.
  2. 2. Secure any leftover substance (blister, bag, foil) and show it to or photograph it for emergency responders — this speeds up the diagnosis.
  3. 3. Calm the person, sit them down, keep the surroundings quiet. Don't let them take anything more.

Don't do

  • Don't try to 'keep them awake' with stimulants or cold showers — this distorts the picture and can worsen symptoms.
  • Don't try to guess what it was and treat accordingly. The hospital makes the diagnosis.
  • Don't throw away the leftover substance, even if there are concerns about police — it can save a life.

“Unknown substance” is a common scenario. Drug checking in Berlin (drugchecking.berlin: vista, Schwulenberatung, Fixpunkt; also Eclipse e.V.) regularly finds:

  • Mistaken identity: “MDMA” sold containing PMMA, cathinone, mephedrone, synthetic cannabinoids
  • Adulterants: cocaine with levamisole, phenacetin; cannabis with synthetics
  • Admixtures: heroin with traces of fentanyl or nitazenes
  • Counterfeit pills: fake benzodiazepines, fake “Oxycontin”

In an acute emergency, identifying the substance is the job of the emergency doctors. The leftover substance or the packaging speeds this up massively — even when police are present (D-002: providing assistance offers legal protection, levelll cannot rule out criminal prosecution).

Preventively: use drug checking regularly. Berlin services:
