Unconscious and breathing shallowly
- 1. Recovery position — now. Head tilted back, mouth open so vomit can drain.
- 2. Call 112. Tell them what was taken if you know. Don't lie out of fear — paramedics need to know what's active.
- 3. Stay there until help arrives. Keep airways clear, monitor breathing.
Not breathing
- 1. Call 112 or have someone call.
- 2. CPR: 30 chest compressions, center of chest, ~5–6 cm deep, 100–120/min.
- 3. Don't stop until paramedics take over. Rescue breaths optional if you know how.
Opioid overdose (Heroin, Fentanyl, Morphine)
Signs: blue-grey lips, pinpoint pupils, shallow breathing, unresponsive.
Naloxone reverses opioid effects immediately. OTC in Germany since 2024 (nasal spray „Nyxoid"). If on hand: use it.
Without naloxone: 112, recovery position, keep airways open, CPR if needed. Stay there.
GHB / GBL — „G-Lock"
Signs: sudden unconsciousness, can't be woken, shallow breathing. Combined with alcohol or benzos: life-threatening.
112 immediately. Recovery position. Never leave alone. There is no antidote — only breathing support.
Half-life ~30 min — if breathing stable, person will sleep it off. But: breathing is the critical question.
Stimulant hyperthermia (MDMA, Cocaine, Speed)
Signs: body temp over 39 °C / 102 °F, seizures, confusion, rash, rapid pulse.
112. Cool the person actively — cold cloths on armpits, groin, neck. Sip water in small amounts (not liters — hyponatremia risk).
Bad trip (psychedelics)
If conscious, responsive, no medical emergency:
- Quiet, familiar place. Dim light.
- Stay present, talk calmly, don't argue.
- Remind: „This passes. You took something. This is the substance."
- If aggressive or self-harming: 112.
